Patient Portal Current patients can communicate with Dr. Taketa-Wong through theCHARM Electronic Health Records Patient Portal. Please follow the directions in the welcome email for current or new patients. If you have forgotten which email you have registered with the CHARM system, you can call or email our office and we can give that to you. We do not have access to any password information, but if you have forgotten your password, you can reset it through the "Forgot Password" link.
